We’ve built our platform to virtually eliminate all administrative burden, with automated assessment scheduling and delivery, real-time data visualizations, and aggregated data for reporting and continuous innovation.

While the process of MBC empowers providers valuable information about how a client is progressing, making sense of that information can still take time. When providers are reviewing multiple assessments, scores and individual responses, the signals most relevant to treatment can be difficult to identify quickly.

Symptom Insights, Focus Areas, and Session Prompts are designed to make that data easier to interpret and act on. Together, these features help providers identify meaningful changes in individual symptoms, maintain visibility into the treatment priorities they’ve established with each client, and bring those insights directly into the therapeutic conversation.

What are Symptom Insights, Focus Areas, and Session Prompts?

These three features work together to help providers move from assessment data to meaningful clinical action.

Symptom Insights automatically analyzes item-level assessment responses and surfaces symptoms that may warrant attention, including symptoms that are severe, deteriorating, improving or related to suicidality.

Focus Areas adds a clinician-directed layer, allowing providers to pin the symptoms that they and their client have identified as treatment priorities. Those symptoms remain easily accessible across sessions, regardless of whether they are currently being surfaced by the Symptom Insights algorithm.

Session Prompts helps providers turn those insights into conversation by providing relevant, data-informed prompts that can support discussion during care.

Together, they help answer three different questions: What should I be aware of? What are we actively working on? What should we talk about?

Why It Matters: Moving Beyond the Total Score

Total assessment scores are an important part of Measurement-Based Care. They provide a standardized way to understand symptom severity and track whether a client is improving over time. But a total score doesn’t always tell the full story.

Two clients with the same PHQ-9 score, for example, may be having very different experiences. One may be struggling primarily with sleep and energy, while another is experiencing feelings of worthlessness or changes in suicidal ideation and those differences matter clinically.

Looking at individual symptoms can help providers better understand what’s driving a client’s overall score, identify meaningful changes that might otherwise be hidden within the total, and have more specific conversations about their experience.

Manually reviewing every item across every assessment isn’t realistic for providers managing busy caseloads. That’s where Symptom Insights, Focus Areas, and Session Prompts come in, addressing these challenges by:

Reducing time spent reviewing data by surfacing the most important symptom-level insights

Simplifying prioritization across multiple assessments and responses

Reducing cognitive load when translating data into in-session conversations

Supporting consistent, high-quality use of MBC across providers and teams

By making data easier to interpret and act on, these features help ensure that measurement is consistently integrated into care, supporting more responsive treatment and stronger client engagement.

Symptom Insights: Automatically Surface Meaningful Changes

Symptom Insights helps providers quickly understand what’s happening beneath an assessment’s total score. Greenspace analyzes individual assessment responses and automatically surfaces symptoms based on clinically relevant signals, including:

Flagged Symptoms: Highlights up to five item-level responses that may require additional attention. These items are prioritized based on clinical urgency, surfacing suicidality risk first, followed by high-severity symptoms and symptoms showing deterioration over time. In just a few seconds, providers can get a focused view of the most clinically concerning symptoms, across all relevant assessments.

Improving Symptoms: Highlights up to five items that have shown the largest improvements, either from the previous response or from baseline, depending on which best reflects meaningful change. By bringing improving symptoms to the forefront, this feature makes it easy to recognize areas where treatment is working, celebrate wins with clients and supervisors, and reinforce positive behaviour and engagement over time.

Instead of reviewing every response individually, providers can quickly see which symptoms may deserve a closer look. This can surface changes that might otherwise be difficult to spot. A client’s overall score may remain relatively stable, for example, while one symptom improves significantly and another begins to deteriorate.

Focus Areas: Keep Treatment Priorities Front and Centre

The symptoms that change most significantly aren’t always the same symptoms a provider and client have decided to prioritize in treatment. Focus Areas gives providers the ability to personalize symptom tracking for each client by pinning the symptoms most relevant to their individual treatment goals.

Providers can add a Focus Area directly from a Flagged or Improving Symptom Insight, as well as from supported item-level assessment responses. Once selected, the symptom appears in a dedicated Focus Areas tab within Insights.

Providers can track up to five Focus Areas for each client, with the latest score, trend and assessment source available at a glance. Unlike automatically generated Symptom Insights, Focus Areas persist across sessions until the provider removes them. This makes it easier to maintain continuity around treatment goals and consistently monitor the symptoms that the provider and client have agreed are important.

When a Focus Area hasn’t been measured in more than four weeks, Greenspace also highlights when it was last assessed, helping providers recognize when it may be time to check in again.

Combining Automated Insights With Clinical Judgment

Symptom Insights and Focus Areas are intentionally complementary. Symptom Insights uses assessment data to automatically identify changes a provider may want to know about. Focus Areas allows the provider to layer their own clinical judgment and knowledge of the client’s treatment goals onto that information.

For example, Greenspace may flag a client’s worsening sleep because of a meaningful change in their response. At the same time, the provider and client may have identified social anxiety as a central treatment target. Rather than choosing between those signals, providers can see both.

The result is a more personalized view of the client’s progress that combines automated analysis with the context providers gain through their ongoing therapeutic relationship.

Session Prompts: Bring the Data Into the Conversation

Session Prompts builds directly on Symptom Insights and Focus Areas by generating data-informed conversation prompts aligned with MBC best practices. These prompts are tied directly to specific itemized responses and their context, allowing providers to seamlessly discuss key symptom areas with their clients. Prompts are generated using instructions developed in-house by Greenspace, in close collaboration with clinical experts.

Session Prompts are designed to support both problem-focused and strength-based discussions, depending on whether the symptom is deteriorating or improving.

Session Prompts includes several capabilities to keep prompts useful and continuously improving:

Prompt regeneration: If a prompt doesn’t fit the context, providers can request a new one in real time.

Inline feedback: Providers can submit feedback directly in product, improving the next suggested prompt.

Session Prompts are designed to support clinical judgment, not to replace it. We encourage providers to view prompts as suggestions or inspiration for ways to bring MBC data into sessions, as a supplement to their own clinical expertise and conversation style.

How They All Work Together in Practice

Imagine a client completes their regular assessments before an upcoming session.

Their overall depression score has changed only slightly, but Greenspace identifies that their sleep difficulties have worsened significantly since their previous assessment. Symptom Insights surfaces that change automatically so the provider doesn’t have to find it manually.

At the same time, the provider and client have been working specifically on feelings of low motivation. The provider has added those symptoms to Focus Areas, keeping them visible across sessions even when they aren’t being automatically flagged.

Before the session, the provider can quickly see both perspectives: what the latest assessment data suggests may need attention and how the client is progressing against their established treatment priorities.

Session Prompts can then help the provider explore relevant changes with the client and bring the assessment data naturally into the conversation.

Instead of simply reviewing scores, the provider has a clearer picture of where the client is improving, where new concerns may be emerging, and how those changes relate to the goals they’re already working toward together.

Making Measurement-Based Care More Personal

Measurement-Based Care is most valuable when data supports, rather than replaces, clinical judgment. Symptom Insights, Focus Areas and Session Prompts are designed around that principle.

Symptom Insights helps providers notice what they might otherwise miss. Focus Areas helps them keep sight of what they and their client have decided matters. Session Prompts helps bring that information into the therapeutic conversation.

Together, they transform assessment data from a collection of scores into a more personalized and actionable view of each client’s care.

As Greenspace continues to build MBC 2.0, the goal is to make it easier for providers to understand what their data is telling them, apply their own clinical context, and use those insights to support more informed, collaborative care.

To learn more about these features and how they can support your MBC implementation, schedule a call with an implementation expert or reach out anytime at info@greenspacehealth.com.

Measurement-Based Care (MBC) at Greenspace has always been about making client-reported data more accessible and actionable in care.
