Care Management

A Sneak Peek into the Platform

Applying the clinical efficacy of Measurement-Based Care and the Stepped Care model to a self-guided, population health setting, our Population Health Hubs set the standard for mental health care by providing people with immediate access to the support they need and meaningfully improving their mental health.

Seamless Resource Navigation

Provide your population with a one-stop, digital-front-door for their mental health, with an intuitive hub featuring all the tools and resources they need to thrive.

Regular Self-Assessments

Brief regualr assessments will help your population better understand their mental health and monitor their well being. The regular check-ins, delivered automatically to users via email or SMS, help keep people engaged in their mental health, ensures high user-retention and reduces deterioration.

Proactive Mental Health Engagement

All users have access to a Wellness Dashboard to monitor their symptom changes week by week. When users can track and understand their mental health outcomes, it helps to improve their wellbeing, prevent the need for further care, and empowers them to self-identify when they are in need of support.

Personalized Care Pathways

Not everyone needs the same type of mental health support. Your Hub will direct people to the care that is most appropriate for their specific needs, including self-guided tools, navigation support from Care Teams, 1:1 therapy, crisis support and more. Empowering people to choose the right service for them helps to reduce wait times and costs of care, and improve overall outcomes.

Evidence-Informed Mental Health Strategy

Aggregate usage and user-reported outcome data is presented in in a dashboard that is fully customizable for your organization. These outcomes ensure your Care Team can proactively support those most at risk and helps to inform your mental health strategy by providing insight into which resources individuals are using and which are most effective in maintaining or improving their mental health.

Care Management

Leverage customizable risk flags to inform Care Management Teams of those most at risk, and proactively reach out to help them access the support they need; whether that is suggesting a behavioral goal to track, scheduling a call to navigate to the most ideal resource or suggesting they access in-person care or crisis resources.

The Economic Impact of a Population Health Hub

Our Population Health Hubs drastically improve resource navigation, increase population engagement with upstream and preventative services, and significantly enhances symptom improvement and population wellness. The result is a full ecosystem of care that meets your population where they’re at in their mental health journey, ensuring that people have timely access to the most appropriate resources for their needs, while reducing wait times and healthcare costs.

Reduce the strain on in-person and downstream services

Due to complex or fractured service navigation, many people's first interaction with mental healthcare is with 1:1 therapy or they wait to access any services until they’re in crisis and in need of high-acuity and high-cost services. Not only does this create longer wait times for services and drastically increase costs of care, it prevents people from proactively accessing upstream services (e.g., Self-Guided iCBT, Group Therapy, Mindfulness Apps etc.) that prevent continued deterioration that can lead to severe mental health challenges. With Risk Flags based on deterioration, symptom severity and suicidal ideation, our Care Management functionality ensures your team is aware of the people most at-risk and can proactively reach out to ensure they access the most appropriate services for their challenges and reduce the demand on higher levels of care.

Fulfill the need for low-cost, self-guided resources

Results from over 250,000 users of our Population Health Hubs show that when alternative self-guided resources are available, 70% of individuals choose to access lower-cost, self-guided resources first instead of 1:1 counselling services. This helps to drastically decrease the cost of care for each individual, as well as improving the overall wellness of the population.

Improve population-level outcomes

Our Population Health Hub outcome data shows that, regardless of the resources users access, the more assessments an individual completes, the higher their symptom improvement. This is similar to the research and evidence surrounding Measurement-Based Care in clinical settings, which we’ve embedded as a foundational component of our Population Health Hubs. It’s clear, from both qualitative user feedback and quantitative outcome data, that providing self-guided users the ability to track and understand their mental health outcomes drastically improves engagement and helps to significantly reduce mental health challenges.

Preventative mental health supports significantly reduce physical and mental healthcare costs.

Without widely available preventative interventions, the likelihood of downstream acute mental health challenges increases, which is associated with higher-cost mental and physical health services. Evidence shows that untreated mental health challenges contribute to a 60% increase in costs for physical health services compared to individuals with the same physical conditions who have accessed evidence-based mental health services. Additionally, about 40% of people with anxiety and depression are also facing a long-term physical health problem, and the cost of managing their physical health is about 50% higher compared to those with the same physical conditions who do not experience mental health challenges. (Clark & Layard, 2014).

Impactful Resource Investments

Leveraging the Hubs customized outcome dashboards and reports empowers teams to access evidence-based insights to identify service gaps and invest in resources and services that best address the needs of the population. As a large or complex organizations or health system, you will be well-equipped with the objective outcome data necessary to iterate and innovate your mental health strategy to improve the impact of your population-level mental health strategy and supports, while reducing the overall cost of mental and physical health services.

FREQUENTLY ASKED QUESTIONS

Categories
How often can users track their progress?

People accessing the resource hub can track their progress by completing a mental wellness self-assessment at their chosen frequency (default frequency is set to every two weeks). They can choose to receive a notification to complete the assessment every week, two weeks, four weeks or eight weeks. Assessment frequency can also be configured based on organization and user preferences or best practices.

How do regular assessments help to enhance user engagement and improve mental health symptoms?

Across our Population Health Hub implementations, usage and outcome data shows that regular self-assessments help to drastically enhance continued mental health engagement and symptom improvement. The average 30-day user retention rate across our Hubs is 43%, which is very promising, especially when compared to the industry average for self-guided mental health tools of between 3-6%. As well, those who return to specifically complete their self-assessments account for 93% of returning users.

It is clear that users return to the Hub in order to complete their self assessments, which helps users stay engaged in preventative care. Additionally, assessment engagement also correlates to overall symptom improvement. Our outcome-data, from over 250,000 users, shows that the more assessments a person completes, the more their wellness improves over time.

How can Hub users interpret their data? Are there resources to support them?

The Population Health Hub presents non-diagnostic mental health data to users, alongside unique insights and prompting questions that help encourage users to reflect on their symptom changes that have occurred since their last assessment. The goal is to spark their curiosity about changes in their results to help them gain a better understanding of their mental health, including what things in their life may be impacting them positively or negatively.

The Hub also recommends resources based on a user’s most recent assessment results. Regular assessment completion helps users understand when a resource is, or is not, working for them and when they may need to step up to a higher level of care (e.g. 1:1 therapy).

How will clinicians and clinical teams be able to leverage data to support users?

Your Population Health Hub will be tailored to your organizations specific implementation. There are three ways in which data can be shared with providers, dependent on your implementation:

  1. If using the Hub in isolation of a Greenspace MBC solution or EHR, user data will be treated as anonymized PHI, meaning only users will have access to their results. In this case, users can choose to export their data and either bring it with them to 1:1 counselling appointments or email it directly to their clinician, in order to help deepen conversations with their therapist. The organization providing the Hub will be able to access and leverage aggregate, anonymized data through their customized organizational dashboard.
  2. For full implementations of Greenspace that include Greenspace’s clinical Measurement-Based Care and/or Intake Platforms, user results can be integrated within their other Greenspace platforms and their EHR so that clinicians and clinic teams can access client results directly within either system.
  3. If your organization is using the Hub in isolation of a Greenspace MBC platform and your clinical team leverages an EHR system to store client outcome data, with their permission, user results can be easily synced with the client record on your EHR and leveraged to inform treatment goals and decisions.

Is there an option for users to access the Hub from a mobile application?

The Hub is accessible via mobile, tablet, or desktop browser and has been optimized to provide a seamless experience across all settings. Greenspace Population Health Hubs also offers the option for a mobile application that can be leveraged to increase Hub accessibility and provide an option for further engagement from your Hubs most active users.

Does Greenspace maintain security practices in line with industry best practices and Canadian standards?

Unless otherwise agreed to by the user for research or clinical purposes, personal information will not be shared, sold, or disclosed to third parties, as described in our Privacy Policy. You can read our full privacy policy here.

Is it secure for users to complete assessments online?

The assessments that are delivered to users (via browser, by email or SMS) don’t contain any personally identifying information or health information. When assessments are completed, the data is sent to the application’s server through secure channels (HTTPS, SSH, etc.). No user information in conjunction with user names is ever sent over unsecured email or other unsecured channels.
