LOFT Community Services: Using Measurement-Based Care to improve outcomes for individuals with complex mental and physical health challenges

LOFT Community Services is one of the largest community service providers in Ontario and we’re excited to welcome them on board as a new Greenspace partner. They specialize in servicing individuals with complex mental health challenges, addictions, dementia and homelessness. While LOFT has always tracked how often their services were being used, they’re eager to use Greenspace’s Measurement-Based Care (MBC) platform to better understand the quality of their services and how to improve them, based off of consistent patient-reported outcomes.

Greenspace’s MBC platform is being implemented in a new care transition program being launched in a partnership between LOFT and Humber River Hospital. They plan to use MBC to measure symptoms, functioning, and overall quality of life for clients who are transitioning from the hospital to supportive housing, as well as within their youth program. The goal is to demonstrate the effectiveness of the services, enhance client engagement, and ultimately improve the quality of life for the population they serve.
