Grand River Hospital’s mental health & addictions programs support individuals of all ages and care needs as they continually strive to positively impact the mental health of their community.
Grand River Hospital recognizes that efficient and consistent tracking of patient symptoms throughout care will have a meaningful influence on improved discharge planning, safe transfer of care to community-based providers, and positive treatment outcomes. Furthermore, they are excited by the ability to make use of Greenspace’s objective data visualization and reporting features to enhance quality improvement processes and provide patients with visibility into their own treatment progress.
